Mounjaro is the brand name for tirzepatide, a weekly injection originally licensed for type 2 diabetes. It is now used off-label for weight loss in adults with obesity or overweight plus weight-related health conditions. Mounjaro mimics two hormones (GLP-1 and GIP) to reduce appetite, slow digestion, improve insulin sensitivity, and support weight loss.

Mounjaro requires a structured dose titration schedule to optimise weight loss outcomes while minimising side effects. The pharmacist prescriber will implement a stepwise increase in dose over the first several months of therapy. Below we outline the standard dosing plan for tirzepatide in weight management, as well as how to handle dose adjustments in practice.
Dose Initiation: All patients start on the lowest dose of 2.5 mg once weekly.
This starter dose is primarily to allow the body to begin adjusting to the medication’s effects.
Note: 2.5 mg is a sub-therapeutic dose for weight loss (it’s not expected to cause much weight reduction on its own), but it is critical for tolerability. Most side effects, especially gastrointestinal ones, occur as the dose increases. Starting low helps patients cope with any nausea or GI upset.
Dose Escalation Schedule: The dose is increased in 2.5 mg increments at minimum 4-week intervals. A typical titration schedule is as follows:

A BMI ≥30 (classified as obese), or
A BMI between 27–30 with weight-related health conditions (e.g. type 2 diabetes, high blood pressure, or sleep apnoea).

Attach a new needle to the pen securely.
Slowly turn the dose knob until you hear two clicks.
Prime the pen by releasing a small amount to remove air bubbles and confirm it’s working.
Set the dosage to ‘1’.
Insert the needle into your chosen injection site, press down the dose knob, and count slowly to five.
Remove the needle and check that the dose counter has returned to ‘0’.
Safely remove the needle and recap the pen.
Aim to take your injection at the same time each week for consistent results—choose a day and time that works best for you.

Is Mounjaro approved for weight loss?
Mounjaro KwikPen is approved for the treatment of adults with type 2 diabetes. It is also authorised for weight management in adults with a BMI of 30kg/m² or above (classified as obesity), and in those with a BMI between 27kg/m² and 30kg/m² (classified as overweight) who have weight-related conditions such as prediabetes, hypertension, dyslipidaemia, or cardiovascular disease.
Who qualifies for tirzepatide?
Adults with a BMI ≥30, or ≥27 with weight-related conditions (diabetes, hypertension, etc.).
Why no form-only consultations?
To meet GPhC requirements and ensure patient safety, all consultations must be interactive – face-to-face or video.
