What is yellow fever?​

Yellow fever (YF) is an acute viral haemorrhagic disease transmitted by infected mosquitoes.

The virus is transmitted via the bite of infected mosquitoes. Monkeys and humans are the hosts of the YF virus.

Once infected with the virus, the mosquito remains infectious for life (two to three months). 

Generally, mosquito species are most active at times of high rainfall, both in the jungle and urban environments. 

YF virus can survive very dry conditions in mosquito eggs, which hatch once a water collection is replenished. This can result in YF outbreaks in urban and rural areas, even during the drier months.

What are the signs & symptoms of yellow fever?

Once contracted, the yellow fever virus incubates in the body for 3 to 6 days. Many people do not experience symptoms, but when these do occur, the most common are fever, muscle pain with prominent backache, headache, loss of appetite, and nausea or vomiting. In most cases, symptoms disappear after 3 to 4 days.

A small percentage of patients, however, enter a second, more toxic phase within 24 hours of recovering from initial symptoms. High fever returns and several body systems are affected, usually the liver and the kidneys. In this phase people are likely to develop jaundice (yellowing of the skin and eyes, hence the name ‘yellow fever’), dark urine and abdominal pain with vomiting. Bleeding can occur from the mouth, nose, eyes or stomach. Half of the patients who enter the toxic phase die within 7 – 10 days.

How can you treat or prevent YF?

 

Treatment of yellow fever:

There is no specific treatment for yellow fever, except supporting treatments in order to relieve symptoms and if needed life support measures.

Usual pain killers, such as paracetamol can be used for body aches and temperature. Ibuprofen, Aspirin and other similar non-steroidal anti-inflammatories should be avoided due to the increased risk of bleeding.

Rest and plenty of fluid are also important.

Infected people should be staying under a mosquito net to reduce further exposure to mosquito bites so that the transmission cycle can be curbed.

 

Yellow fever prevention:

There are two methods to prevent yellow fever:

  1. Mosquito control and bite avoidance – click here for more information.
  1. Yellow fever vaccine – yellow fever is prevented by an extremely effective vaccine, which is safe and affordable.

The vaccine provides effective immunity for 80-100% of vaccinated people within the initial 10 days and for 99% of vaccinated people within the first 30 days.

Schedule:

Single-dose at least 10 days before travel.

 

Booster:

Usually, a booster dose is not required.

A single dose of yellow fever vaccine (Stamaril) is enough to provide sustained immunity and lifelong protection against yellow fever disease.

A booster dose would only be recommended if you were previously vaccinated in one of the following circumstances:

  • while pregnant
  • less than two years old
  • had a weakened immune system
  • whilst infected with HIV
  • before undergoing a bone marrow transplant

 

Validity:

From 11 July 2016 the certificate of vaccination against yellow fever is valid for the life of the person vaccinated. This lifetime validity applies automatically to all existing and new certificates, beginning 10 days after the date of vaccination.

Yellow fever vaccination certificate

Some countries require proof of vaccination certificate when visiting those countries or passing through areas where yellow fever is found, even if there’s no risk of picking up the infection in these countries.

The certificate is known as an International Certificate of Vaccination or Prophylaxis (ICVP).

You’ll be given a certificate when you’re vaccinated at a yellow fever vaccination centre (YFVC).

If you are excluded from YF vaccination, then our clinician would be able to provide you with an exemption letter (fee applicable), which may be accepted by officials in countries that usually require a vaccination certificate.

YF vaccine not suitable

People who are usually excluded from vaccination include:

  • babies under 9 months of age – babies who are 6 to 9 months old may sometimes be vaccinated if the risk of getting yellow fever is high

  • people over the age of 60

  • pregnant women – except during a yellow fever outbreak when the risk of infection is high;

  • people with severe allergies to egg protein; and

  • people with severe immunodeficiency due to symptomatic HIV/AIDS or other causes, or who have a thymus disorder.
